Indications
- Bronchial asthma
- Bronchospasm
- Chronic obstructive pulmonary disease (COPD)
- Pulmonary disease with spastic bronchial component
Pharmacology
Doxophylline is a novel bronchodilator. It selectively inhibits phosphodiesterase 4, relaxes bronchial smooth muscle, and has decreased affinities toward adenosine A1 and A2 receptors.
Dosage & Administration
- Elderly: 200 mg tablet two or three times daily
- Adults: 400 mg tablet two or three times daily
- Children:
- >12 years of age: 10 ml syrup or 200 mg tablet two or three times daily
- 6-12 years of age: 6-9 mg/kg body weight two times daily
Interaction
Avoid administering with other xanthine preparations, limit caffeine consumption, and exercise caution with sympathomimetic drugs and concurrent administration of various medications.
Contraindications
Contraindicated in individuals with known hypersensitivity to the drug or other xanthine derivatives, patients with acute myocardial infarction, hypotension, and during lactation.

Pregnancy & Lactation
Use of the drug during pregnancy should be evaluated carefully on the basis of the risk-benefit ratio. Contraindicated during lactation.
Precautions & Warnings
Caution in patients with cardiac disease, hypertension, in the elderly, in patients with severe hypoxemia, hyperthyroidism, chronic corpulmonale, congestive heart failure, liver disease, peptic ulcer, renal impairment, and other specified conditions. Monitoring of the concentration of the blood levels of the drug is recommended for the control of the therapeutic range.
Overdose Effects
Severe cardiac arrhythmias and tonic-clonic seizure may occur. There is no specific antidote, so a symptomatic treatment of cardiovascular collapse should be instituted.
