Indications
- Hypertension
- Treatment of essential hypertension
- Chronic heart failure (CHF)
- Treatment of stable mild and moderate chronic heart failure in elderly patients
Pharmacology
- Inhibition of beta adrenergic receptors
- Lack of intrinsic sympathomimetic and membrane stabilizing activity
- Decreased heart rate
- Decreased myocardial contractility
- Diminution of tonic sympathetic outflow
- Suppression of renin activity
- Vasodilation and decreased peripheral vascular resistance
- Metabolism by glucuronidation and hydroxylation
- Plasma protein binding of approximately 98%
- Absorption similar to oral solution
- Mean peak plasma concentrations approximately 1.5 to 4 hours post-dosing
- Predominant excretion in urine and feces
Dosage & Administration
- Recommended starting dose of 5 mg once daily
- Dose can be increased at 2-week intervals up to 40 mg
Interaction
- Careful use with myocardial depressants or inhibitors of AV conduction
- Not to be combined with other beta-blockers
- Cautious co-administration with CYP2D6 inhibitors
Contraindications
- Severe bradycardia
- Greater than first degree heart block
- Cardiogenic shock
- Decompensated cardiac failure
- Sick sinus syndrome
- Severe hepatic impairment
- Hypersensitivity to any component of the product
Side Effects
- Headache
- Nausea
- Bradycardia
Precautions & Warnings
- Abrupt cessation of therapy
- Caution in cardiac failure patients
- Not studied in patients with recent Ml
- Not for patients with bronchospastic diseases
- Closely monitored perioperatively
- Potential masking of hypoglycemia manifestations
- Potential masking of hyperthyroidism clinical signs
- Caution in patients with peripheral vascular disease
- Caution in concomitant use with non-dihydropyridine calcium channel blockers
Use in Special Populations
- Caution in patients with severe renal impairment
- Caution in patients with moderate hepatic impairment
- Increased risk of anaphylactic reactions
