Indications

  • Postoperative pain
  • Colic and spastic pain
  • Cancer pain
  • Joint pain
  • Neck and back pain
  • Pain associated with osteoporosis

Pharmacology

Centrally acting synthetic analgesic compound. Inhibits reuptake of neurotransmitters- serotonin and noradrenaline. Modifies transmission of pain impulses by activating serotonergic pathways and noradrenergic pathways. Mediated via stimulation of mu-opioid receptors and indirect modulation of central monoaminergic inhibitory pathways.

Dosage & Administration

  • Usual doses: 50 to 100 mg every 4-6 hours
  • Initial dose for acute pain: 100 mg
  • Initial dose for chronic painful conditions: 50 mg
  • Sustained Release Capsule or Tablet: one every 12 hours
  • Injection: 50-100 mg every 4-6 hours
  • Suppository for adults: 100 mg every 6 hours

Interaction

Contraindicated with monoamine oxidase inhibitors. Carbamazepine may increase metabolism.

Contraindications

Hypersensitivity to Tramadol. Acute intoxication with alcohol, hypnotics, centrally acting analgesics, opioids or psychotropic drugs.

Pregnancy & Lactation

Safe use not established in pregnancy. Tramadol crosses placenta. Not recommended during breastfeeding.

Precautions & Warnings

  • Respiratory depression risk
  • Not recommended for opioid-dependent patients
  • Caution with concomitant CNS depressants
  • Caution with concomitant MAO inhibitors
  • Caution with increased intracranial pressure or head injury

Use in Special Populations

Children from 1 year: 1-2 mg/kg body weight. Avoid suppository in children under 14 years.
