- Selective ß1 blocker
- Reduces heart rate and cardiac output
- High bioavailability (>90%)
- Extensively distributed
- Metabolized via oxidative pathways
- Renally eliminated
- High therapeutic reliability
- Elimination half-life of 10-12 hours
Dosage & Administration
- Starting dose of 5 mg once daily
- Increase to 10 mg if necessary
- May be increased to 20 mg once daily
- Dose titration interval of 2 weeks
- No established safety and effectiveness in children
- Caution in patients with renal or hepatic impairment
Interaction
- Not recommended with other β-blocking agents
- Monitor closely when combined with catecholamine-depleting drugs
- Use with care in combination with centrally active antihypertensive agents
- Caution when coadministered with antiarrhythmic agents or calcium channel blockers
Contraindications
- Cardiogenic shock
- Overt heart failure
- Second or third degree A-V block
- Right ventricular failure secondary to pulmonary hypertension
