Indications
- Treatment of all forms of dementia of the Alzheimer's type
- Treatment of other types of dementia
Pharmacology
- NMDA receptor antagonist
- Low to moderate affinity for NMDA receptors
- Oral administration
- Linear pharmacokinetics
- Excreted unchanged in urine
- Terminal elimination half-life of 60-80 hours
- Well absorbed with peak concentrations reached in 3-7 hours
- Low plasma protein binding (45%)
- Hepatic metabolism
- Excretion as polar metabolites in urine
- Minimal CYP-450 enzyme involvement
Dosage & Administration
- Recommended maintenance dose of 20 mg/day for adults and older patients
- Titration from 5 mg per week over 3 weeks to achieve maintenance dose
- Dosage adjustments for renal impairment
- No dosage adjustment needed for mild or moderate hepatic impairment
- Not recommended for use in children under 18 years
Interaction
- Enhanced effects of L-dopa, dopaminergic agonists, and anticholinergics
- Reduced effects of barbiturates and neuroleptics
- Possible modification of effects of antispasmodic agents
- Not to be used with specific medications
