Indications
- Treatment of dementia of the Alzheimer's type
- Indicated for other types of dementia
Pharmacology
- NMDA receptor antagonist
- Low to moderate affinity for NMDA receptors
- Well absorbed after oral administration
- Linear pharmacokinetics over therapeutic dose range
- Excreted predominantly unchanged in urine
- Terminal elimination half-life of 60-80 hours
- Peak concentration reached in 3-7 hours after oral administration
- Low plasma protein binding (45%)
- Partially hepatic metabolism
- Predominantly excreted as polar metabolites in urine
- No significant role of the hepatic microsomal CYP-450 enzyme system in metabolism
Dosage & Administration
- Recommended maintenance dose for adults and older patients: 20 mg/day
- Upward titration with 5 mg per week over 3 weeks to achieve maintenance dose
- Dosage adjustment in renal impairment
- No dosage adjustment needed in mild or moderate hepatic impairment
- Not recommended for use in children under 18 years
Interaction
- Enhanced effects with concomitant L-dopa, dopaminergic agonists, and anticholinergics
- Reduced effects with barbiturates and neuroleptics
- Modification of effects with antispasmodic agents (dantrolene, baclofen)
Contraindications
- Known hypersensitivity to Memantine Hydrochloride or excipients used in the formulation

- Bladder infections
- Increased sexual drive
- Possible increase in probability of epileptic seizures
Pregnancy & Lactation
- Pregnancy Category B
- Use during pregnancy only if potential benefit justifies potential risk to the fetus
- No adequate studies in pregnant women
- Caution during lactation
Precautions & Warnings
- Recommended administration and dose escalation for caregivers
- Monitoring of kidney function in patients with kidney dysfunction
- Not systematically evaluated in patients with a seizure disorder
- No risk of carcinogenesis, mutagenesis and impairment of fertility
- Alteration of reaction time with potential impact on driving and operating machinery
