Indications
- Hypertension treatment
- Chronic stable angina treatment
- Hypercholesterolemia treatment
- Reduction of risk of nonfatal MI
- Reduction of risk of nonfatal stroke
- Adjunct to diet for hypercholesterolemia
- Reduction of peripheral vascular resistance
- Reduction in blood pressure
- Treatment of confirmed or suspected vasospastic angina
- Risk reduction of hospitalization due to angina
- Reduction of risk of a coronary revascularization procedure
Pharmacology
- Contains Amlodipine Besilate BP equivalent to 5 mg Amlodipine
- Contains Atorvastatin calcium INN equivalent to 10 mg Atorvastatin
- Peripheral arterial vasodilator
- Reduction in peripheral vascular resistance
- Synthetic lipid-lowering agent
- Inhibitor of 3-hydroxy-3-methyl-glutaryl-coenzyme A (HMG-CoA) reductase
Dosage & Administration
- Usual initial antihypertensive oral dose of Amlodipine: 5 mg once daily
- Maximum dose of Amlodipine: 10 mg once daily
- Recommended starting dose of Atorvastatin: 10 mg daily
- Dosage range of Atorvastatin: 10 to 80 mg once daily
- May be administered with or without food
Interaction
- Increased risk of myopathy with concurrent administration of certain drugs
- Possible potentiation of other antihypertensive effects by Amlodipine
- No effect on the pharmacokinetics of Atorvastatin, Digoxin, Warfarin or Cyclosporine by Amlodipine
Contraindications
- Amlodipine contraindicated in patients with known hypersensitivity
- Atorvastatin contraindicated in hypersensitivity to any component of the medication
- Contraindicated in active liver disease or unexplained persistent elevations of serum transaminases exceeding three times the upper limit of normal
Side Effects
- Possible acute hypotension with Amlodipine
- Caution with all calcium channel blockers in patients with heart failure
- Possible rhabdomyolysis with acute renal failure secondary to myoglobinuria with Atorvastatin
Pregnancy & Lactation
- Safety not established in pregnancy
- Use of HMG-CoA reductase inhibitors not recommended during breastfeeding
Precautions & Warnings
- Warning of increased angina and/or myocardial infarction with calcium channel blocker therapy
- Caution with administering Amlodipine in patients with severe aortic stenosis
- Caution with HMG-CoA reductase inhibitors in patients with liver dysfunction
- Elevated serum creatine phosphokinase levels should be considered in the differential diagnosis of chest pain in patients on Atorvastatin therapy
Use in Special Populations
- Safety and efficacy of Atorvastatin not established in children
- Efficacy and safety in older patients using recommended doses similar to that seen in the general population
