Indications

  • Prophylaxis of angina pectoris

Pharmacology

  • Nitroglycerin causes relaxation of vascular smooth muscle
  • Produces a vasodilator effect on both peripheral arteries and veins

Dosage & Administration

  • Dosage should be adjusted according to individual patient's requirement and response
  • Recommended adult dosage: one Nitroglycerin 2.6 mg sustained release tablet or capsule in morning and evening
  • Should be taken on an empty stomach

Interaction

  • May increase antihypertensive properties of vasodilators, calcium antagonists, beta-adrenergic blockers
  • Concomitant use with tricyclic antidepressants and alcohol may cause high blood pressure
  • Concomitant use with phosphodiesterase type 5 (PDE5) inhibitors causes fall in blood pressure
  • Aspirin decreases the clearance and enhances the hemodynamic effects
  • Reduces pharmacologic effects of heparin when used concomitantly
  • Increases the bioavailability of dihydroergotamine

Contraindications

  • Known hypersensitivity to nitroglycerin, other organic nitrates, or nitrites
  • Contraindicated in patients with acute myocardial infarction, marked anemia, head trauma, cerebral hemorrhage, or closed angle glaucoma

Side Effects

  • Headache at the onset of treatment
  • Tachycardia
  • Postural hypotension and syncope
  • Cyanosis and methemoglobinemia

Pregnancy & Lactation

  • No evidence related to the safety of nitrates in pregnancy and lactation
  • Should be administered to pregnant women and nursing mothers only if considered essential by the physician

Precautions & Warnings

  • Use with caution in patients predisposed to closed-angle glaucoma
  • Abrupt discontinuation may lead to exacerbation of symptoms
  • Dosage reduction should be gradual over several days when discontinuing long-term treatment

Overdose Effects

  • Peripheral vasodilation with a fall in blood pressure and reflex tachycardia
  • Monitoring of cardiac function and general supportive measures should be used in the case of overdosage
  • In life-threatening situations, administration of vasopressors should be considered
