Indications
- Replacement therapy in hypothyroidism of any aetiology
- Suppress TSH levels in the presence of goitres, nodules, and after thyroid cancer treatment
- Suppress goitrogenic effects of other drugs
- Diagnostic aid in suppression tests
Pharmacology
- Contains synthetic Levothyroxine (T4)
- About 30% of T4 is converted to Triiodothyronine (T3)
- TBG is the major carrier of T4
- Long half-life of T4 in circulation, 6-7 days
- Liver is the major site of degradation of thyroid hormones
Dosage & Administration
- Initial starting dose for adults: 25-50 mcg/day
- Dosage adjusted in 12.5-25 mcg increments for primary hypothyroidism
- Different dosages for severe hypothyroidism, pituitary or hypothalamic hypothyroidism, and age groups
Interaction
- Concurrent use with tri/tetracyclic antidepressants may increase therapeutic and toxic effects
- Potential interaction with sertraline and antidiabetic/insulin therapy
- Reduced serum digitalis glycoside levels in hyperthyroidism
Contraindications
- Untreated subclinical or overt Thyrotoxicosis
- Acute Myocardial Infarction
- Uncorrected Adrenal failure
Side Effects
- Adverse reactions primarily those of hyperthyroidism
- General, CNS, musculoskeletal, cardiovascular, respiratory, gastrointestinal, and dermatologic side effects
Pregnancy & Lactation
- Pregnancy Category A
- Levothyroxine requirements may increase during pregnancy
- Caution with nursing women, but generally needed for normal lactation
Precautions & Warnings
- Adrenocortical insufficiency in pituitary hypothyroidism
- Careful dosing in patients with cardiovascular disease or severe hypothyroidism
- Adjustment of dosage with marked change in body weight
- Monitoring blood levels of T3 and T4
- Establishing correct dosage by monitoring serum levels of TSH
Overdose Effects
- Signs and symptoms of hyperthyroidism
- Cerebral embolism, shock, coma, and death reported
- Reduce or temporarily discontinue dose if signs of overdosage occur
