Indications
- Prophylaxis and chronic treatment of asthma
- Acute prevention of Exercise-Induced Bronchoconstriction (EIB)
- Relief of symptoms of Allergic Rhinitis (AR): Seasonal & Perennial Allergic Rhinitis
Pharmacology
- Selective and orally active leukotriene receptor antagonist
- Inhibition of cysteinyl leukotriene receptor (CysLT1)
- Correlation with the pathophysiology of asthma & allergic rhinitis
- Effects on airway edema, muscle contraction, and inflammatory process
Dosage & Administration
- Adult dosage: Montelukast 10 mg tablet once daily
- Pediatric dosage: Montelukast 5 mg or 4 mg tablet once daily, or 4 mg oral granules once daily
- Use in pediatric patients aged 6 months to 14 years
- No dosage adjustment for hepatic or renal insufficiency
- Similar pharmacokinetic profile in elderly and younger adults
Interaction
- Administered with other asthma therapies with no increase in adverse reactions
- No clinically important effects on the pharmacokinetics of specified drugs
- Concomitantly used with commonly prescribed drugs without evidence of adverse interactions
- Potential for interaction with cytochrome P450 enzyme inducers like phenobarbital or rifampin
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Common: Diarrhea, fever, gastrointestinal discomfort, headache, nausea, vomiting, skin reactions, upper respiratory tract infection
- Uncommon: Akathisia, anxiety, arthralgia, asthenia, depression, dizziness, drowsiness, dry mouth, malaise, muscle complaints, seizures, sleep disorders
- Rare: Angioedema, disorientation, hallucination, hepatic disorders, memory loss, palpitations, suicidal tendencies, tremor
Pregnancy & Lactation
- Crosses the placenta in rats and rabbits
- Caution during pregnancy and lactation
- Use only if clearly needed during pregnancy
- Exercise caution when given to a nursing mother
Precautions & Warnings
- Not indicated for reversal of bronchospasm in acute asthma attacks
- Appropriate rescue medication advised during acute exacerbations of asthma
- Caution during reduction of inhaled corticosteroid dose
- Not for monotherapy in the treatment of exercise induced bronchospasm
- Avoidance in patients with known aspirin sensitivity
Overdose Effects
- No adverse experiences in the majority of overdosage reports
- Usual supportive measures recommended in the event of overdose
