Indications

• Prophylactic treatment of urinary tract infections
• Suppressive treatment of urinary tract infections
• Long-term therapy for recurring urinary tract infections
• Eradication of infection using other antimicrobial agents
• Reducing development of drug-resistant bacteria
• Selection of antibacterial therapy based on culture and susceptibility information
• Empiric selection of antibacterial therapy based on local epidemiology and susceptibility patterns

Pharmacology

• Methenamine component hydrolyzed to formaldehyde in acid urine
• Hippuric acid keeps urine acid
• Active against E. coli, enterococci, and staphylococci
• Inhibition of urea-splitting organisms such as Proteus and Pseudomonas
• Demonstrable antibacterial activity in urine within 1/2 hour of ingestion
• Over 90% excretion of methenamine moiety in urine within 24 hours
• Rapid absorption and excretion of hippurate moiety
• Importance of action in older patients or those with renal impairment

Dosage & Administration

• 1 tablet twice daily for adults and pediatric patients over 12 years
• Fractional to 1 tablet twice daily for pediatric patients 6 to 12 years
• Greater antibacterial activity in acid urine
• Restriction of alkalinizing foods and medications
• Supplemental acidification of urine if necessary
• Monitoring of therapy efficacy by repeated urine cultures

Contraindications

• Renal insufficiency
• Severe hepatic insufficiency
• Severe dehydration
• Use with sulfonamides due to potential formation of insoluble precipitate in urine

Pregnancy & Lactation

• Safe use not established in early pregnancy
• Suggested safety in last trimester
• No adverse effects on fetus in animal studies
• Interference with laboratory tests of urine estriol when using acid hydrolysis

Precautions & Warnings

• Avoid use in absence of proven or strongly suspected bacterial infection
• Maintain acid pH of urine, especially in infections due to urea-splitting organisms
• Recommended liver function studies, especially in patients with liver dysfunction

Use in Special Populations

• Elderly - cautious dose selection, especially with decreased hepatic, renal, or cardiac function and concomitant disease or drug therapy
• Renal insufficiency and severe hepatic insufficiency - contraindicated
