Description
Rostatin is indicated for Heterozygous Hypercholesterolemia, Homozygous Hypercholesterolemia, Mixed Dyslipidemia, and Primary prevention of cardiovascular disease. It functions by inhibiting HMG-CoA reductase and increasing hepatic LDL receptors. The dose range is 5-40 mg once daily, with specific doses for pediatric and HoFH patients. It can be taken with or without food at any time of day. It has contraindications for hypersensitivity, liver disease, and pregnancy, and is generally well tolerated with side effects like headache and myalgia. The safety in pregnant women has not been established, and precautions should be taken for skeletal muscle effects and liver enzyme abnormalities. The drug should be stored below 30°C temperature, protected from light and moisture.
Indications
- Heterozygous Hypercholesterolemia (Familial and Non familial)
- Homozygous Hypercholesterolemia (Familial)
- Mixed Dyslipidemia (Fredrickson Type IIa and IIb)
- Primary prevention of cardiovascular disease
Pharmacology
Selective and competitive inhibitor of HMG-CoA reductase, increases the number of hepatic LDL receptors and inhibits hepatic synthesis of VLDL and LDL particles
Dosage
- Dose range: 5-40 mg once daily
- HoFH: Starting dose 20 mg/day
- Pediatric patients with HeFH: 5-10 mg/day for patients 8 to less than 10 years age, and 5-20 mg/day for patients 10 to 17 years of age
- Pediatric patients with HoFH: 20 mg/day for patients 7 to 17 years of age
Administration
Can be taken with or without food, at any time of day
Interaction
- Cyclosporine: Combination increases exposure, dose should be limited to 5 mg once daily
- Gemfibrosil: Combination should be avoided, if used together, dose should be limited to 10 mg once daily
- Lopinavir/Ritonavir or atazanavir/ritonavir: Combination increases exposure, dose should be to 10 mg once daily
- Coumarin anticoagulants: Combination prolongs INR, stable INR should be achieved prior to starting Rostatin
- Concomitant lipid-lowering therapies: Use with fibrates and niacin products may increase the risk of skeletal muscle effects
Contraindications
- Known hypersensitivity to product components
- Liver disease, unexplained persistent elevations in hepatic transaminase levels
- Pregnant women and women who may become pregnant
- Nursing mothers

Pregnancy & Lactation
Safety in pregnant women not established, excretion in human milk unknown
Precautions & Warnings
- Skeletal muscle effects increase with 40 mg dose, advanced age (>65 year), hypothyroidism, renal impairment and combination use with certain other drugs
- Liver enzyme abnormalities and monitoring: Persistent elevations in hepatic transaminases can occur, should be monitored before and during treatment
- Use in children: Safety and effectiveness not established
