Description
Reduction of cholesterol saturation of bile, promotion of dissolution of gallstones, reduction of cholesterol secretion into bile, and increased bile acid secretion rate during treatment without a reduction in phospholipids.
Pharmacology
- Main function: Decrease in secretion of cholesterol in bile
- Mechanisms:
- Reduces cholesterol absorption
- Suppresses liver cholesterol synthesis
- Does not inhibit bile acid synthesis
- Other functions:
- Protects cholangiocytes against cytotoxicity of hydrophobic bile acids
- Stimulates hepatobilliary secretion
- Protects hepatocytes against bile acid-induced apoptosis
- Absorption:
- Absorption rate: 60-80%
- Maximum serum concentration time: 30-150 minutes
- Absorption location: Upper intestine
- Excretion:
- Route: Urinary
- Biologic half life: 3.5-5.8 days
Dosage and administration
- Dissolution gallstones: 8-12 mg/kg/day as single night time dose or in divided doses
- Primary billiary cirrhosis: 10-15 mg/kg/day in 2-4 divided doses
- Acute viral hepatitis: 600 mg/day
- Alcoholic fatty liver: 300 mg/day
- Primary sclerosing cholangitis: 25-30 mg/kg/day
- Dissolution gallstones nash: 13-15 mg/kg/day
Interaction
Should not be used with drugs that increase bile cholesterol. Concomitant administration with bile-acid binding drugs including antacids, charcoal, and cholestyramine should be avoided, as this may reduce the effectiveness of therapy.
Contraindications
- Non-functioning gall-bladder
- Calcified and pigmented gallstones
- Inflammatory bowel disease
