Description

Reduces cholesterol saturation of bile, promotes dissolution of gallstones, reduces cholesterol secretion into bile, increases bile acid secretion rate without reduction in phospholipids

Pharmacology

  • Mechanisms:
    • Reduces cholesterol absorption
    • Suppresses liver cholesterol synthesis
    • Does not inhibit bile acid synthesis
  • Benefits:
    • Protects cholangiocytes against cytotoxicity of hydrophobic bile acids
    • Stimulates hepatobilliary secretion
    • Protects hepatocytes against bile acid-induced apoptosis
  • Absorption:
    • Time to Peak Serum Concentration: 30 to 150 minutes
    • Rate: 60-80%
    • Extraction from Portal Blood by Liver: Conjugated with amino acid - glycine or taurine
  • Excretion:
    • Into Hepatic Bile Ducts: After conjugation with amino acid
    • Circulation: Small quantities
    • Urine: Very small amounts
  • Half Life: 3.5-5.8 days

Dosage & Administration

  • Dissolution of Gallstones: 8-12 mg/kg/day
  • Primary Billiary Cirrhosis: 10-15 mg/kg/day
  • Acute Viral Hepatitis: 600 mg/day
  • Alcoholic Fatty Liver: 300 mg/day
  • Primary Sclerosing Cholangitis: 25-30 mg/kg/day
  • Dissolution of Gallstones & Non-Alcoholic Steato Hepatitis: 13-15 mg/kg/day

Interaction

Should not be used with drugs that increase bile cholesterol or concomitantly with bile-acid binding drugs

Contraindications

  • Non-functioning gall-bladder
  • Calcified and pigmented gallstones
  • Inflammatory bowel disease

Pregnancy & Lactation

  • Pregnancy Category: B
