Indications
- Severe extensive psoriasis treatment
- Palmo-plantar pustular psoriasis treatment

Pharmacology
- Retinoid
- Aromatic analogue of vitamin A
- Regulation of RNA/DNA synthesis
- Modulation of factors influencing epidermal proliferation
- Modification of glycoprotein synthesis
- Modulation of the immune response
Dosage & Administration
- Initial daily dose of 25mg or 30mg for 2 to 4 weeks
- Daily dosage of 25-50mg for 6 to 8 weeks
- Maximum dosage of 75mg/day
- Starting dose of 10mg for Darier’s disease
- Daily dosage for children is about 0.5mg/kg
- Higher doses up to 35mg/day may be necessary in some cases for limited periods
Interaction
- Concurrent intake with ethanol leads to the formation of etretinate
- Contraindicated with methotrexate, tetracyclines, or vitamin A
- Reduces the protein binding of phenytoin
- May affect the protein binding of anticoagulants of the coumarin type (warfarin)
- Low dose progesterone-only products may be inadequate for contraception
Contraindications
- Highly teratogenic, not to be used by pregnant women or those who might become pregnant
- Contraindicated in severe hepatic or renal impairment
- Contraindicated in cases of hypersensitivity to the preparation or other retinoids
- Patients with rare glucose-galactose malabsorption should not use this medicine
Side Effects
- Most patients experience dose-related side effects
- Toxic dose is close to therapeutic dose
- Initial worsening of psoriasis symptoms may occur at the beginning of treatment
Pregnancy & Lactation
- Contraindicated in pregnant women or nursing mothers
- Not to be used by women who might become pregnant during or within 2 years of cessation of treatment
Precautions & Warnings
- Exceptionally high risk of giving birth to a deformed child if taken before or during pregnancy
- Enhanced effects of UV light, patients should avoid excessive sunlight exposure
- Hepatic function, serum cholesterol, and serum triglycerides should be regularly monitored
- Retinoids can alter glucose tolerance, blood sugar levels should be checked
- Patients should be warned of the possibility of alopecia and decreased night vision
Overdose Effects
- Manifestations of acute Vitamin A toxicity include severe headache, vertigo, nausea, drowsiness, irritability and pruritus
- Specific treatment is unnecessary due to low acute toxicity
