Indications
- Prophylaxis and chronic treatment of asthma
- Acute prevention of Exercise-Induced Bronchoconstriction (EIB)
- Relief of symptoms of Allergic Rhinitis (AR): Seasonal & Perennial Allergic Rhinitis
Pharmacology
- Selective and orally active leukotriene receptor antagonist
- Inhibits the cysteinyl leukotriene receptor (CysLT1)
- Correlated with asthma & allergic rhinitis pathophysiology
Dosage and Administration
- Dosage for adults and adolescents 15 years of age and older: Montelukast 10 mg tablet once daily
- Dosage for pediatric patients 6 to 14 years of age: Montelukast 5 mg tablet once daily
- Dosage for pediatric patients 2 years to 5 years of age: Montelukast 4 mg tablet once daily
- Dosage for pediatric patients 6 months to 5 years of age: Montelukast 4 mg oral granules once daily
- No dosage adjustment for hepatic insufficiency and renal insufficiency
- Similar pharmacokinetic profile in elderly and younger adults
Interaction
- No clinically important effects with theophylline, prednisone, prednisolone, oral contraceptives, terfenadine, digoxin, warfarin, thyroid hormones, sedative hypnotics, non-steroidal anti-inflammatory agents, benzodiazepines, decongestants
- Phenobarbital decreased the AUC of Montelon by approximately 40%
- Appropriate clinical monitoring for potent cytochrome P450 enzyme inducers
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Common: Diarrhoea, fever, headache, nausea, vomiting, skin reactions, upper respiratory tract infection
- Uncommon: Anxiety, arthralgia, abnormal behavior, depression, drowsiness, dry mouth, seizure, sleep disorders
- Rare: Angioedema, disorientation, hallucination, hepatic disorders, memory loss, suicidal tendencies, tremor
Pregnancy & Lactation
- Crosses the placenta in rats and rabbits
- Use during pregnancy only if clearly needed
- Caution in nursing mothers
Precautions & Warnings
- Not for use in reversal of bronchospasm in acute asthma attacks
- Appropriate rescue medication should be available
- Continuation during acute exacerbations of asthma
- Should not be abruptly substituted for inhaled or oral corticosteroids
- Not to be used as monotherapy for the treatment of exercise induced bronchospasm
- Avoidance in patients with known aspirin sensitivity
- Not shown to truncate bronchoconstrictor response to aspirin in aspirin-sensitive asthmatic patients
Overdose Effects
- No adverse experiences in majority of reports
- Usual supportive measures recommended in case of overdose
