Pharmacology

  • Mode of Action: Selective ß1 adrenergic receptor blocker
  • Effect on Heart Rate: Reduces heart rate
  • Effect on Cardiac Output: Reduces cardiac output
  • Effect on Arterial Hypertension: Decreases arterial hypertension
  • Lipid Metabolism: Does not affect cholesterol fraction including HDL-cholesterol
  • Pharmacokinetic Properties:
    • Absorption and Bioavailability: High absorption rate and absolute bioavailability of 88%
    • Distribution: Extensively distributed with medium distribution volume of 3.51L/kg
    • Metabolism: Primarily metabolized via oxidative pathways
    • Elimination: Balanced between renal elimination and hepatic metabolism

Dosage and Administration

  • Adult: Initial dose of 5 mg once daily, may be increased up to 20 mg once daily
  • Children: Safety and effectiveness not established
  • Patients With Renal or Hepatic Impairment: Initial daily dose should be 5 mg

  • Contraindicated with:
    • Other β-blocking Agents
    • Catecholamine-Depleting Drugs
  • Special Caution with:
    • Centrally Active Antihypertensive Agents
    • Antiarrhythmic Agents
    • Calcium Channel Blockers

Contraindications

Cardiogenic shock, Overt heart failure, Second or third degree A-V block, Right ventricular failure secondary to pulmonary hypertension, Sinus bradycardia
