Indications

  • Heterozygous Hypercholesterolemia (Familial and Non familial)
  • Homozygous Hypercholesterolemia (Familial)
  • Mixed Dyslipidemia (Fredrickson Type IIa and IIb)
  • Primary prevention of cardiovascular disease

Pharmacology

Selective and competitive inhibitor of HMG-CoA reductase, increases hepatic LDL receptors, inhibits hepatic synthesis of VLDL

Dosage

5-40 mg once daily, 40 mg dose for patients not reaching LDL-C goal with 20 mg, specific doses for pediatric patients

Administration

Can be taken with or without food, at any time of day

Interaction

  • Cyclosporine increases exposure, Gemfibrosil combination should be avoided, Lopinavir/Ritonavir increases exposure, Coumarin anticoagulants prolong INR, Concomitant lipid-lowering therapies increase risk of skeletal muscle effects

Contraindications

  • Known hypersensitivity to product components
  • Liver disease with unexplained persistent elevations in hepatic transaminase levels
  • Pregnant women and women who may become pregnant
  • Nursing mothers

  • Asthenia
  • Abdominal pain
  • Nausea

Pregnancy & Lactation

Safety in pregnant women not established, excretion in human milk unknown

Precautions & Warnings

  • Skeletal muscle effects increase with 40 mg dose, advanced age, hypothyroidism, renal impairment, combination use with specific drugs
  • Liver enzyme abnormalities and monitoring required
