Indications
- Treats hypertension
- Lowers blood pressure
- Reduces risk of cardiovascular events
- May be used alone or in combination with other antihypertensive agents
Pharmacology
- Dihydropyridine calcium antagonist
- Inhibits transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle
- Selective effect on vascular smooth muscle cells
- Greater effect on vascular smooth muscle cells than on cardiac muscle cells
- 1:1 racemic mixture of levamlodipine and dextro amlodipine
- Pharmacologically active isomer is levamlodipine
- Oral administration
- Takes 6-12 hours to reach peak concentration (Tmax)
- Bioavailability of 64-90%
- Volume of distribution similar to amlodipine (21 L/kg)
- Metabolized to inactive metabolites
- Metabolism primarily mediated by CYP3A4
- 60% eliminated in urine
- 10% eliminated as unmetabolized drug
Dosage & Administration
- Oral route of administration
- Usual initial dose is 2.5 mg once daily
- Maximum dose is 5 mg once daily
- Adult and elderly patients may start with 1.25 mg once daily
- Dosage needs to be adjusted according to blood pressure goals
- Pediatric patients ages 6-17 years may take 1.25 mg to 2.5 mg once daily
- Titration needed with a waiting period between titration steps
- Requires assessment and frequent monitoring
Interaction
- CYP3A inhibitors increase systemic exposure to amlodipine
- CYP3A inducers effects not quantitatively available
- Co-administration with simvastatin increases the systemic exposure of simvastatin
- May increase systemic exposure of cyclosporine or tacrolimus when co-administered
- Can be taken with or without food
- Avoid grapefruit products and natural licorice
Contraindications
- Contraindicated in patients with known sensitivity to amlodipine
Side Effects
- Common: Difficulty in breathing, Dizziness, Reflex Tachycardia, Hypotension, Abdominal Pain, Edema
- Rare: Diarrhea, Back pain, Headache, Fatigue, Nausea
Pregnancy & Lactation
- Limited data available for use in pregnant women
- No adverse effects on the breastfed infant observed
Precautions & Warnings
- Possible symptomatic hypotension
- Risk of increased angina or myocardial infarction
- Titrate slowly in patients with severe hepatic impairment
Overdose Effects
- Excessive peripheral vasodilation
- Marked hypotension
- Possibly reflex tachycardia
