Indications
- Treatment of hypertension
- Lowering blood pressure
- Initial therapy for patients needing multiple antihypertensive agents
Pharmacology
- Dihydropyridine calcium channel blocker
- Inhibits transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle
- Peripheral arterial vasodilator
- Blocks vasoconstrictor and aldosterone-secreting effects of angiotensin II
Dosage & Administration
- Initial dose of 5/20 mg once daily
- Titration up to maximum of 10/40 mg once daily
- Special starting dose for elderly patients
Interaction
- Potential attenuation by NSAIDs including selective COX-2 inhibitors
- Close monitoring of blood pressure, renal function, and electrolytes
Contraindications
- Co-administration with Aliskiren in patients with diabetes

- Vasodilation in severe aortic stenosis
- Increased risk in severe obstructive coronary artery disease

- No studies in patients with renal impairment
- Not recommended in hepatically impaired patients
