Description
- Simvastatin is a medication used to lower cholesterol levels and reduce the risk of coronary heart disease. It belongs to the class of statins, which are anti-cholesterol drugs.
Indications
- Primary hypercholesterolemia (type IIa and IIb)
- Coronary heart disease
- Elevated plasma cholesterol level
Mode of Action
- Acts as a cholesterol lowering agent
- Inhibition of HMG-CoA reductase activity
- Increases LDL receptor density on liver cells
- Increases removal of LDL cholesterol from plasma
- Reduces formation of LDL from VLDL
Dosage & Administration
- Usual starting dose of 10 mg/day in the evening
- Adjustment of dosage at intervals of not less than four weeks
- Maximum dosage of 40 mg daily in the evening
- Starting dose in hypercholesterolemia is 5-10 mg once a day
- Starting dose in patients with coronary heart disease is 20 mg once a day
- No significant renal excretion
Interaction
- Concomitant administration with digoxin
- Enhancement of anticoagulant effect of coumarin derivatives
- No clinically significant adverse interactions with ACE inhibitors, beta-blockers, calcium channel blockers, diuretics, and NSAIDs
Contraindications
- Active liver disease
- Pregnant and breastfeeding women
- Women of childbearing age without adequate contraception
- Hypersensitivity to any component
- Homozygous familial hypercholesterolemia with complete absence of LDL receptors

- Avoid pregnancy during and for one month after treatment
