Description

  • Simvastatin is a medication used to lower cholesterol levels and reduce the risk of coronary heart disease. It belongs to the class of statins, which are anti-cholesterol drugs.

Indications

  • Primary hypercholesterolemia (type IIa and IIb)
  • Coronary heart disease
  • Elevated plasma cholesterol level

Mode of Action

  • Acts as a cholesterol lowering agent
  • Inhibition of HMG-CoA reductase activity
  • Increases LDL receptor density on liver cells
  • Increases removal of LDL cholesterol from plasma
  • Reduces formation of LDL from VLDL

Dosage & Administration

  • Usual starting dose of 10 mg/day in the evening
  • Adjustment of dosage at intervals of not less than four weeks
  • Maximum dosage of 40 mg daily in the evening
  • Starting dose in hypercholesterolemia is 5-10 mg once a day
  • Starting dose in patients with coronary heart disease is 20 mg once a day
  • No significant renal excretion

Interaction

  • Concomitant administration with digoxin
  • Enhancement of anticoagulant effect of coumarin derivatives
  • No clinically significant adverse interactions with ACE inhibitors, beta-blockers, calcium channel blockers, diuretics, and NSAIDs

Contraindications

  • Active liver disease
  • Pregnant and breastfeeding women
  • Women of childbearing age without adequate contraception
  • Hypersensitivity to any component
  • Homozygous familial hypercholesterolemia with complete absence of LDL receptors

  • Avoid pregnancy during and for one month after treatment
