Pharmacology
Inhibits CNS neuronal uptake of serotonin, blocks serotonin reuptake, enhances serotonin actions on 5HT1A autoreceptors
Dosage & Administration
- Adults: Starting dose is 50 mg, titrated up to 300 mg/day
- Pediatric Population: Starting dose for ages 8-17 is 25 mg, titrated up to 200 mg/day
- Elderly or Hepatically Impaired Patients: May require modified initial and titration doses
Interaction
- Inhibits several cytochrome P450 isoenzymes (CYP1A2, CYP2C9, CYP3A4, CYP2C19)
- Carbamazepine, Sumatriptan, Tacrine, Tricyclic Antidepressants (TCAs), Tryptophan, Diltiazem, Propranolol or metoprolol: Specific interactions and precautions
Contraindications
- Coadministration of tizanidine, thioridazine, alosetron, pimozide
- Serotonin Syndrome and MAOIs: Avoid coadministration with MAOIs
Side Effects
- Common reactions in adult OCD and depression patients
- Various specific reactions in patients with OCD and pediatric patients with OCD
Pregnancy & Lactation
Consider both potential risks and benefits when treating a pregnant woman, secreted in human breast milk
Precautions & Warnings
- Use with caution in patients with specific medical history
- May impair the performance of skilled tasks (driving)
