Pharmacology
Inhibition of CNS neuronal uptake of serotonin, enhancement of serotonin actions on 5HT1A autoreceptors, virtually no affinity for various receptors
Dosage & Administration
- Adults: Recommended starting dose of 50 mg, titration within a range of 100 to 300 mg/day
- Pediatric Population: Recommended starting dose for ages 8-17 years of 25 mg, titration within a range of 50 to 200 mg/day
- Elderly or Hepatically Impaired Patients: Decreased clearance observed, may require modification of initial dose and subsequent titration
Interaction
- Inhibition of CYP1A2, CYP2C9, CYP3A4, and CYP2C19
- Elevated carbamazepine levels and symptoms of toxicity with coadministration
- Rare reports of weakness, hyperreflexia, and incoordination following use of an SSRI and sumatriptan
- Increased tacrine C max and AUC with coadministration resulting in nausea, vomiting, sweating, and diarrhea
- Significantly increased plasma TCA levels with coadministration of TCAs
- Severe vomiting with coadministration of tryptophan
- Bradycardia with coadministration of diltiazem
- Dose reduction and cautious titration advised with coadministration of propranolol or metoprolol
Contraindications
- Coadministration of tizanidine, thioridazine, alosetron, pimozide
- Contraindicated with MAOIs and linezolid or intravenous methylene blue
