Pharmacology

  • Inhibition of CNS neuronal uptake of serotonin
  • Enhancement of actions of serotonin on 5HT1A autoreceptors
  • Affinity for binding to σ1 receptors

Dosage & Administration

  • Recommended starting dose for adults: 50 mg
  • Maximum therapeutic benefit not to exceed 300 mg per day
  • Pediatric starting dose (ages 8-17 years): 25 mg
  • Maximum dose in children up to age 11: 200 mg/day
  • Elderly or Hepatically Impaired Patients: Adjusted initial dose and subsequent titration

Interaction

  • Inhibition of cytochrome P450 isoenzymes (CYP1A2, CYP2C9, CYP3A4, CYP2C19)
  • Elevated carbamazepine levels and symptoms of toxicity with coadministration
  • Rare reports of weakness, hyperreflexia, and incoordination with SSRI and sumatriptan use
  • Increased tacrine C max and AUC with coadministration
  • Significantly increased plasma TCA levels with coadministration
  • Severe vomiting with tryptophan coadministration
  • Bradycardia with diltiazem coadministration
  • Dose reduction and cautious titration with propranolol or metoprolol coadministration

Contraindications

  • Coadministration of tizanidine, thioridazine, alosetron, pimozide
  • Serotonin Syndrome and MAOIs
  • Avoid use with linezolid or intravenous methylene blue within 14 days

Precautions & Warnings

  • Use with caution in patients with a history of mania, seizures, suicide, concomitant ECT, cardiac disease, diabetes mellitus, angle-closure glaucoma, bleeding disorders, hepatic and renal impairment
  • May impair performance of skilled tasks (driving)
