Indications

Pharmacology
- Atypical psychotropic agent
- Belongs to dibenzothiazepine derivatives
- Antagonist at multiple neurotransmitter receptors (serotonin 5HT1A, 5HT2, dopamine D1, D2, histamine H1, adrenergic α1, α2)
- No appreciable affinity at cholinergic muscarinic and benzodiazepine receptors
Dosage & Administration
- Administered twice daily for acute and chronic psychoses
- Administered twice daily for manic episodes associated with bipolar disorder
- Administered once daily at bedtime for depressive episodes associated with bipolar disorder
- Usual effective dose range of 300-450 mg/day for acute and chronic psychoses
- Usual effective dose range of 400-800 mg/day for manic episodes associated with bipolar disorder
- Usual dose of 300 mg/day for depressive episodes associated with bipolar disorder
- Maintenance dose of 400-800 mg/day for bipolar I disorder in combination with mood stabilizers
Interaction
- Caution with medicines known to cause electrolyte imbalance
- Caution with medicines known to increase QT interval
- Co-administration with thioridazine or carbamazepine increases clearance
- Co-administration with phenytoin increases clearance
Contraindications
- Hypersensitivity to Quetiapine
Side Effects
- Somnolence
- Dizziness
- Dry mouth
- Withdrawal symptoms
- Elevations in serum triglyceride levels
- Elevations in total cholesterol (predominantly LDL cholesterol)
- Decreases in HDL cholesterol
- Weight gain
- Decreased haemoglobin
- Extrapyramidal symptoms
Pregnancy & Lactation
- Safety and efficacy not established in human pregnancy
- Use during pregnancy only if benefits justify potential risks
- Avoid breastfeeding while taking Quetiapine
Precautions & Warnings
- Increased risk of suicidal thoughts
- Caution in patients with known cardiovascular disease
- Caution in patients with cerebrovascular disease
- Caution in patients predisposed to hypotension
- Caution when treating patients with a history of seizures
- Consider dose reduction if signs of tardive dyskinesia appear
- Discontinue if Neuroleptic Malignant Syndrome is suspected
- Exercise caution in patients with cardiovascular disease or family history of QT prolongation
- Monitor for severe neutropenia
- Monitor for increases in blood glucose and hyperglycemia
- Avoid use in children and adolescents below 18 years of age
Use in Special Populations
- Use with caution in the elderly
Overdose Effects
- Survival reported in acute overdoses up to 30 grams
- No specific antidote
- Consider multiple drug involvement
- Intensive care procedures recommended
