Indications
- Neuropathic pain associated with diabetic peripheral neuropathy (DPN)
- Postherpetic neuralgia (PHN)
- Adjunctive therapy for the treatment of partial-onset seizures in patients 1 month of age and older
- Fibromyalgia
- Neuropathic pain associated with spinal cord injury
Pharmacology
- Structural derivative of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA)
- Binds with high affinity to the alpha2-delta site
- Anti-nociceptive effects
- Antiseizure effects
Dosage
- Maximum recommended dose of 100 mg three times a day for adults with DPN
- Recommended dose of 75 to 150 mg two times a day for PHN in adults
- Recommended dose range of 300 to 450 mg/day for fibromyalgia in adults
- Recommended dose range of 150 to 600 mg/day for neuropathic pain associated with spinal cord injury in adults
Administration
- Oral route of administration
- Can be taken with or without food
- Pregabalin CR tablet should be administered after an evening meal
- Should be swallowed whole and not be split, crushed, or chewed
Contraindications
- Contraindicated in patients with known hypersensitivity to Pregabalin or any of its components
Side effects
- Most common side effects in adults: dizziness, somnolence, dry mouth, edema, blurred vision, weight gain, and thinking abnormal
- Most common side effects in pediatric patients: increased weight and increased appetite
Pregnancy and lactation
- No adequate and well-controlled studies in pregnant women
- Small amounts detected in the milk of lactating women
Precautions and warnings
- Angioedema can occur and may be associated with life-threatening respiratory compromise
- Discontinue immediately in cases of angioedema or hypersensitivity reactions
- Increase the risk of suicidal thoughts or behavior
- Respiratory depression may occur when used with concomitant CNS depressants
- May cause dizziness and somnolence, impairing the ability to drive or operate machinery
- May cause peripheral edema
- Cautious coadministering with thiazolidinedione antidiabetic agents
Use in special populations
- Safety and effectiveness in pediatric patients have not been established
- Safety and effectiveness in pediatric patients below the age of 1 month have not been established
Overdose effects
- Signs and symptoms of overdose: reduced consciousness, depression/anxiety, confusional state, agitation, restlessness, seizures, heart block
- No specific antidote
- General supportive care indicated including monitoring of vital signs
