Indications
- Primary hyperlipidemia and mixed dyslipidemia
- Hypertriglyceredemia
- Dysbetalipoproteinemia
- Homozygous familial hypercholesterolemia (HoFH)
- Slowing the progression of atherosclerosis
- Risk reduction of Myocardial Infarction (MI), stroke and arterial revascularization procedures
- Pediatric patients 10 to 17 years of age with heterozygous familial hypercholesterolemia (HeFH)
Pharmacology
- Selective and competitive inhibitor of HMG-CoA reductase
- Increases hepatic LDL receptors
- Inhibits hepatic synthesis of VLDL and LDL particles
Dosage & Administration
- Usual dose range: 5-20 mg/day
- Maximum recommended dose: 20 mg/day
- Starting dose for homozygous familial hypercholesterolemia: 20 mg once daily
- Initiation of therapy with 5 mg once daily for Asian patients
- Dosing for patients with severe renal impairment: 5-10 mg once daily
Interaction
- Cyclosporine increases exposure
- Gemfibrosil combination should be avoided
- Lopinavir/Ritonavir or atazanavir/ritonavir combination increases exposure
- Coumarin anticoagulants combination prolongs international normalized ratio (INR)
- Use with fibrates and niacin products may increase the risk of skeletal muscle effects
Contraindications
- Known hypersensitivity to product components
- Liver disease with unexplained persistent elevations in hepatic transaminase levels
- Pregnant women and women who may become pregnant
- Nursing mothers

Pregnancy & Lactation
- Prescribed with caution to women of childbearing age
- Discontinuation of therapy if pregnancy occurs
- Excretion in human milk uncertain
Precautions & Warnings
- Prescribed with caution in patients with predisposing factors for myopathy
- Discontinued if markedly elevated creatinine kinase levels occur
- Liver enzyme tests should be performed before and during therapy

- Symptomatic and supportive measures
- Hemodialysis does not significantly enhance clearance
