Indications
- Relief of migraine headache pain
- Analgesic for post-operative pain

Pharmacology
- Belongs to fenamate group
- Inhibitor of cyclooxygenase enzyme
- Inhibits synthesis of thromboxane (TX) B2
- Inhibits synthesis of prostaglandin (PG) E2
- Direct antagonistic action on prostaglandin receptors
- Readily absorbed from GI tract
- Peak plasma concentration in 60-90 min
- Bioavailability of 85%
- Metabolized in the liver
- Excreted in urine and feces
Dosage & Administration
- Adult dosage for acute migraine attacks: 200 mg when symptoms appear, may be repeated once after 1-2 hours
- Adult dosage for mild to moderate pain: 100-200 mg tid
- Dose adjustments needed for renal impairment
- Avoid in severe renal impairment
- Pediatric dosage regimen not established
- Should be taken with food
- Water intake during or immediately after meals
Interaction
- Rate of absorption affected by Metoclopramide and Magnesium hydroxide
- Rate of absorption decreased with Aluminium hydroxide
- Increased risk of bleeding with anticoagulants and other NSAIDs
- Decreased antihypertensive response to loop diuretics, B-blockers, and ACE Inhibitors
- Co-administration increases plasma concentration of Lithium, Methotrexate, and cardiac glycosides
- Increases risk of nephrotoxicity with ACE inhibitors, Ciclosporin, Tacrolimus, or diuretics
Contraindications
- Active gastrointestinal bleeding
- Active gastrointestinal ulceration
- History of gastrointestinal bleeding related to previous NSAID therapy
- Severe heart failure
