Description
Tolfenic acid is a potent inhibitor of cyclooxygenase enzyme and is used for relieving the pain of migraine headache, post-operative pain, and fever. It is readily absorbed from the GI tract and has a bioavailability of 85%. It should be taken with food and water during or immediately after meals. This medicine is contraindicated in cases of active gastrointestinal bleeding, ulceration, severe heart failure and during the third trimester of pregnancy. It should be used with precaution in patients with asthma, cardiovascular diseases, peptic ulceration, hypertension, and liver or renal impairment.
Dosage
- Acute migraine attacks: 200 mg when symptoms appear may be repeated once after 1-2 hour
- Mild to moderate pain: 100-200 mg tid

Interaction
- Increases Absorption with:
- Metoclopramide
- Magnesium hydroxide
- Decreases Absorption with:
- Aluminium hydroxide
- Risks:
- Bleeding with anticoagulants and other NSAIDs
- Decreased antihypertensive response to loop diuretics, B-blockers and ACE Inhibitors
- Increased plasma concentration of Lithium Methotrexate and cardiac glycosides
- Increases Risk of Nephrotoxicity with:
- ACE inhibitors
- Ciclosporin
- Tacrolimus
- Diuretics
Contraindications
- Active gastrointestinal bleeding
- Gastrointestinal ulceration
- Severe heart failure
- History of gastrointestinal bleeding related to previous NSAID therapy
- History of gastrointestinal haemorrhage
- History of gastrointestinal perforation related to previous NSAID therapy
- History of recurrent gastrointestinal ulceration
