Indications
- Erectile Dysfunction (ED)
- Benign Prostatic Hyperplasia (BPH)
- Both Erectile Dysfunction and signs and symptoms of Benign Prostatic Hyperplasia
Pharmacology
- Tadalafil is a selective phosphodiesterase type 5 (PDE5) inhibitor
- Increase in cGMP in smooth muscle cells
- Causes smooth muscle relaxation and increased blood flow into the corpus cavernosum
- Inhibition of PDE5 in smooth muscles of the prostate and bladder wall
- Improves blood flow to the urinary tract and widens the opening of the bladder neck
Dosage & Administration
- Recommended starting dose for Erectile Dysfunction: 10 mg
- Maximum dosing frequency: once daily
- Effective for up to 36 hours
- Recommended dose for Benign Prostatic Hyperplasia: 5 mg daily
- Recommended dose for Combined Erectile Dysfunction and Benign Prostatic Hyperplasia: 5 mg daily
Interaction
- May interact with Nitrates, Alpha adrenergic blockers, Antihypertensives, Alcohol, Antacids, Ketoconazole, Ritonavir, Erythromycin, Itraconazole, Grapefruit juice, other HIV protease inhibitors, Rifampin, Carbamazepine, Phenytoin, Phenobarbital
Contraindications

Side Effects
- Common side effects: Headache, Dyspepsia, Back pain, Myalgia, Nasal pharyngitis, Nasal congestion
- Rare side effects: Change in Color Vision, Sudden vision loss, Hearing loss, Stevens-Johnson Syndrome, Exfoliative dermatitis, Angina, Stroke, Myocardial infarction, Severe hypotension, Tachycardia
Pregnancy & Lactation
- Tadalafil assigned to pregnancy category B by the USFDA
- Recommended for use during pregnancy only if benefit outweighs risk
- No data on excretion of Tadalafil in human milk
- Caution advised when administering tadalafil to nursing women
