Indications
- Treatment of symptomatic anemia associated with chronic kidney disease (CKD) in adult patients
Description
- First-in-class orally administered HIF-PH inhibitor
- Promotes erythropoiesis through increasing endogenous production of erythropoietin
- Improves iron regulation
- Overcomes EPO-suppressive effects of inflammation on hemoglobin synthesis and red blood cell production by downregulating hepcidin
Pharmacology
- Orally bioavailable hypoxia-inducible factor prolyl hydroxylase inhibitor (HIF-PHI)
- Binds to and inhibits HIF-PH
- Increases HIF activity to enhance erythropoiesis
- Reduces hepcidin expression
- Improves iron availability
- Boosts hemoglobin levels
- Regulates expression of genes required for erythropoiesis and iron metabolism
Dosage & Administration
- Orally administered three times per week
- Individualized dose to achieve and maintain target Hb levels
- Different starting doses for patients not on erythropoiesis-stimulating agent treatment and those switching from it
- Dose adjustment guidelines
- Methods of administration with or without food
Interaction
- Potential drug-drug interactions with phosphate binders and other products containing multivalent cation, gemfibrozil, probenecid, and certain substrates
- Recommendations for adjusting Renofix dose based on specific drug interactions
Side Effects
- Common adverse reactions include hypertension, vascular access thrombosis, diarrhea, peripheral edema, hyperkalemia, and nausea
Pregnancy & Lactation
- Contraindicated during pregnancy and breast-feeding
Precautions & Warnings
- Caution in use for patients with pre-existing risk factors for thrombotic vascular events, patients with a history of seizures, serious signs and symptoms of infection, or liver disorder
- Not recommended for pregnant women or children
- Not recommended for use in patients with severe hepatic impairment
Use in Special Populations
- Not indicated for use in children
- Not recommended for use in patients with severe liver dysfunction
Overdose Effects
- Symptoms of increased heart rate and excessive hemoglobin concentrations
- Treatment includes dose reduction or interruption of Renofix, not removed by dialysis
