Indications
- Treatment of adult patients with symptomatic anemia associated with chronic kidney disease (CKD)
Description
- Orally administered HIF-PH inhibitor
- Promotes erythropoiesis
- Increases endogenous production of erythropoietin
- Improves iron regulation
- Overcomes EPO-suppressive effects of inflammation on hemoglobin synthesis and RBC production
Pharmacology
- Orally bioavailable
- Hypoxia-inducible factor prolyl hydroxylase inhibitor (HIF-PHI)
- Binds to and inhibits HIF-PH
- Prevents HIF breakdown and promotes HIF activity
- Increases endogenous erythropoietin production
- Reduces expression of hepcidin
- Improves iron availability
- Boosts hemoglobin (Hb) levels
Dosage & Administration
- Orally administered
- Taken three times per week
- Individualized dose to achieve and maintain target Hb levels of 10 to 12 g/dL
- Usual starting dose of 50 mg three times weekly for patients not on erythropoiesis-stimulating agent treatment
- Missed dose management
Interaction
- Potential drug-drug interactions with phosphate binders, gemfibrozil, probenecid, and statins
- Recommendations for timing and dose adjustments

Pregnancy & Lactation
- Contraindicated during pregnancy and breastfeeding
Precautions & Warnings
- May initiate thrombotic vascular events
- Caution in patients with pre-existing risk factors for TVE
- Not to be administered in cases of serious infection or liver disorder
Use in Special Populations
- Not indicated in children
- Not recommended for use in patients with severe hepatic impairment

- Increased heart rate
- Likely increase in hemoglobin concentration by overdosage
