Indications

  • Treatment of hypertension
  • May be used alone or in combination with other antihypertensive agents

Pharmacology

  • Formation of angiotensin II
  • Vasoconstriction
  • Stimulation of aldosterone synthesis and release
  • Cardiac stimulation
  • Renal reabsorption of sodium
  • AT 1 receptor blocking
  • AT 2 receptor affinity
  • No effect on ACE (kininase II)
  • Inhibition of negative regulatory feedback on renin secretion

Dosage & Administration

  • Individualized dosage
  • Usual starting dose of 20 mg once daily
  • Increased to 40 mg if further blood pressure reduction is required
  • No dosage adjustment for elderly patients
  • Consider lower starting dose for patients with possible volume depletion
  • May be administered with or without food
  • No established safety and effectiveness in pediatric patients

Interaction

  • No significant drug interactions with digoxin or warfarin reported
  • Bioavailability not significantly altered by antacids
  • No inhibition or induction of P450 enzymes
  • Potential renal function deterioration with NSAID co-administration
  • Attenuation of antihypertensive effect by NSAIDs

Contraindications
