Indications
- Palliative treatment of advanced carcinoma of the breast or endometrium
- Treatment of anorexia, cachexia, or unexplained significant weight loss in patients with AIDS and cancer
Pharmacology
- Synthetic, antineoplastic, and progestational drug
- Inhibition of pituitary gonadotrophin production
- Antineoplastic action on carcinoma of the breast
- Plasma half-life of 33 to 38 hours
- Peak plasma concentrations occur 2 to 3 hours after a single oral dose
- 66% excreted in urine and 20% in feces
Dosage & Administration
- 160 mg/day for breast cancer
- 40-320 mg/day in divided doses for endometrial carcinoma
- Recommended adult initial dosage of 800 mg/day (20 ml/day) for oral suspension
Interaction
- Possible interactions with concomitant medications not investigated
- Estrogens and Progestogens may cause fluid retention
Contraindications
- History of hypersensitivity to Megestrol Acetate or any component of the formulation
- Known or suspected pregnancy
Side Effects
- Weight gain associated with increased appetite
- Thromboembolic phenomena including thrombophlebitis and pulmonary embolism
- Glucocorticoid effects
- Nausea, dyspnea, hyperglycemia, alopecia, hypertension, mood changes, rash
Pregnancy & Lactation
- Pregnancy Category D
- Not recommended during the first four months of pregnancy
- Very small amounts (approximately 0.1%) excreted in mother's milk
- Nursing should be discontinued during treatment
Precautions & Warnings
- Close surveillance for patients with recurrent or metastatic cancer
- Use with caution in patients with a history of thromboembolic disease
- Exacerbation of preexisting diabetes with increased insulin requirements
Use in Special Populations
- Safety and effectiveness in pediatric patients not established
- Insufficient data available for patients 65 years of age and older to determine differences in response
Overdose Effects
- No serious unexpected side effects reported from studies involving dosages as high as 1600 mg/day
