Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • NSAID with analgesic & antipyretic properties
  • Prostaglandin synthesis inhibition
  • Proton pump inhibitor
  • Specific inhibition of the H^+/k^+ -ATPase in the gastric parietal cell
  • Reduction of gastric acidity

Dosage

  • Adult dosage: 375 mg naproxen/20 mg esomeprazole or 500 mg naproxen/20 mg esomeprazole twice daily
  • Adolescent dosage: 375 mg naproxen/20 mg esomeprazole or 500 mg naproxen/20 mg esomeprazole for weight >50 kg, 375 mg naproxen/20 mg esomeprazole for weight 38-50 kg

Administration

  • Tablet should not be split, chewed, crushed or dissolved
  • To be taken at least 30 minutes before meals

Interaction

  • Reduced antihypertensive effect with concomitant use of NSAIDs and ACE inhibitors, diuretics & beta-blockers
  • Increased bleeding risk with concomitant use of warfarin
  • Interference with drug absorption with Esomeprazole

Contraindications

  • Hypersensitivity to any component
  • History of asthma, urticaria or allergic-type reactions after taking aspirin or other NSAIDs
  • Use during peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Decreased occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events
  • Possible small increased risk of arterial thrombotic events
  • Gastrointestinal adverse events: peptic ulcers, perforation or GI bleeding, nausea, vomiting, diarrhoea, abdominal pain, etc.
  • Oedema, hypertension & cardiac failure reported
  • Possible reduction of vitamin B12 absorption

Pregnancy & Lactation

  • Not recommended during first and second trimester of pregnancy
  • Contraindicated during third trimester of pregnancy
  • Not recommended during breastfeeding
  • NSAIDs may impair female fertility

Precautions & Warnings

  • Avoid combination with other NSAIDs
  • Minimize dose and duration to control symptoms
  • Increased risk of serious NSAID-related adverse events in older people
  • Risk-factors for NSAID related gastro-intestinal complications
  • Increased frequency of adverse reactions in older people

Use in Special Populations

  • High unbound plasma fraction of naproxen in elderly
  • Not recommended in patients with moderate to severe renal impairment
  • Possible dose reduction in patients with mild to moderate hepatic impairment

Overdose Effects

  • Symptoms include lethargy, dizziness, abdominal discomfort, renal dysfunction, respiratory depression
  • Symptoms of deliberate esomeprazole overdose are transient
  • Hemodialysis does not decrease plasma concentration of Naproxen
  • No specific antidotes for either Naproxen or Esomeprazole overdose
