Description
Lipitin is indicated as an adjunct to diet to reduce elevated total cholesterol, LDL cholesterol, apolipoprotein B (Apo-B) and triglycerides levels in various diseases.
Dosage
Adults: Usually 10 mg once daily; may be increased to max. 80 mg once daily. Children (10-18 years): Initially 10 mg once daily, increased if necessary at intervals of at least 4 weeks.
Indications
- Reduce total cholesterol and LDL cholesterol in patients with familial hypercholesterolaemia
- Reduce elevated cholesterol and triglycerides in patients with mixed dyslipidemia
- Treatment of patients with elevated serum triglyceride levels in hypertriglyceridaemia
- Reduce cardiac ischaemic events in patients with coronary artery disease
- Reduce total and LDL-cholesterol concentrations in patients with hypercholesterolemia associated with diabetes mellitus or renal transplantation
Mode of action
Selectively inhibits HMG-CoA reductase, lowering plasma cholesterol and lipoprotein levels by inhibiting cholesterol synthesis in the liver.
Absorption
Rapidly absorbed after oral administration; maximum plasma concentrations occur within 1 to 2 hours.
Distribution
Mean volume of distribution of Atorvastatin is approximately 381 liters. Atorvastatin is 98% bound to plasma proteins.
Metabolism
Extensively metabolized to ortho- and parahydroxylated derivatives and various beta-oxidation products.
Excretion
Eliminated primarily in bile following hepatic and/or extra-hepatic metabolism.
Interaction
- Increases myopathy risk:
- Concurrent administration with cyclosporine, fibric acid derivatives, niacin, erythromycin, azole antifungals
- Antacid: Decreases plasma concentrations of atorvastatin by approximately 35%, but LDL-C reduction is not altered
- Colestipol: Decreases plasma concentrations of atorvastatin by approximately 25%, but LDL-C reduction is greater
- Digoxin: Steady-state plasma digoxin concentrations increased by approximately 20% when coadministered with atorvastatin
- Erythromycin: Plasma concentrations of atorvastatin increased approximately 40% with coadministration of atorvastatin and erythromycin
- Oral contraceptives: Increased AUC values for norethindrone and ethinyl estradiol by approximately 30% and 20%
- Warfarin: No clinically significant effect on prothrombin time when administered to patients receiving chronic warfarin treatment
Contraindications
- Hypersensitivity to any component of this medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

Precautions warnings
- Liver effects: Liver function tests should be performed before initiation of treatment and periodically thereafter; use with caution in patients who consume substantial quantities of alcohol or have a history of liver disease.
- Elderly use: Plasma concentrations are higher in healthy elderly subjects; greater degree of LDL-lowering compared to younger adults
- Renal hepatic insufficiency: Does not influence plasma concentrations or LDL-C reduction; dose adjustment not necessary
Pregnancy lactation
- Pregnancy: Contraindicated during pregnancy; safety not established; should be suspended for the duration of pregnancy
- Lactation: Not known whether excreted in human milk; contraindicated during breastfeeding
Overdose effects
Specific treatment not available for atorvastatin overdose; symptomatically and supportive measures instituted, as required; liver function tests should be performed and serum CK levels monitored.
