Description
- Immunosuppressive agent
- Uncompetitive and reversible inhibitor of inosine monophosphate dehydrogenase (IMPDH)
- Inhibits de novo pathway of guanosine nucleotide synthesis
- Cytostatic effects on lymphocytes
- Prevents acute rejection of kidney and heart allotransplantation
- Decreases antibody production
Pharmacology
- Potent, selective, uncompetitive, and reversible inhibitor of IMPDH
- Inhibition of de novo pathway of guanosine nucleotide synthesis
- More potent cytostatic effects on lymphocytes
- Blocks DNA and RNA synthesis in B and T lymphocytes
- Decreases intracellular pools of guanosine triphosphate (GTP) and deoxyguanosine triphosphate (dGTP)
Dosage & Administration
- Standard dosage for prophylaxis of renal rejection in adults and children
- Standard dosage for prophylaxis of cardiac rejection in adults and children
- Standard dosage for prophylaxis of hepatic rejection in adults and children
- Standard dosage for treatment of first or refractory renal rejection in adults and children
- Standard dosage for induction therapy and maintenance therapy in adults and children
- Use in combination with corticosteroids
- Introduction and adjustment of doses according to clinical response
- Therapeutic drug monitoring for prevention of sub-therapeutic exposure
Interaction
- Caution with concomitant administration of antacids, azathioprine, cyclosporin, rifampin, sevelamer, cholestyramine, acyclovir, metronidazole, hormonal contraceptives
Contraindications
- Hypersensitivity to mycophenolic acid or its excipients
- Pregnancy
- Breastfeeding

- Urinary tract infection
- Herpes simplex
- Vomiting
- Higher frequency of certain types of infections and opportunistic infections
Pregnancy & Lactation
- Contraindicated in pregnancy and breastfeeding
Precautions & Warnings
- Increased risk of new or reactive viral infections
- Blood dyscrasias including pure red cell aplasia (PRCA)
- Serious GI tract complications
- Increased risk of developing lymphomas and other malignancies
- Avoidance of live vaccines during treatment
- Avoid blood donation during therapy
Use in Special Populations
- Increased risk of adverse events in geriatric population

- Possible over suppression of the immune system
- Increased susceptibility to infections
