Indications
- Single infestations of hookworm (Ancylostoma, Necator)
- Mixed infestations of roundworm (Ascaris), threadworm (Enterobius), whipworm (Trichuris), strongyloides, tapeworm, opisthorchi, hydatid
Description
- Benzimidazole anthelmintic
- Active against most nematodes and some cestodes
- Used in the treatment of intestinal nematode infections
- Used in higher doses for the treatment of hydatid disease
- Exhibits vermicidal, ovicidal, and larvicidal activity
- Inhibitory effect on tubulin polymerization
Pharmacology
- Broad spectrum anthelmintic
- Exhibits vermicidal, ovicidal, and larvicidal activities
- Blocks glucose uptake in susceptible helminths
- Depletes energy level of helminths for survival
- Binds and inhibits parasite tubulin polymerization
- Extensively metabolized, probably in the liver
- Poorly absorbed from the gastrointestinal tract
- Rapid extensive first-pass metabolism
- Principal metabolite albendazole sulphoxide has anthelmintic activity
- Excretion in urine with other metabolites
Dosage & Administration
- Recommended dose for adults & children over 2 years: 400 mg as a single dose for certain infections
- Recommended dose for strongyloidiasis or taeniasis: 400 mg daily for 3 consecutive days
- Recommended dose for hydatid disease: 400 mg twice daily for patients weighing over 60 kg
- Recommended dose for cystic echinococcosis: 28 days course may be repeated after 14 days without treatment
- Recommended dose for alveolar echinococcosis: cycles of 28 days of treatment followed by 14 days without treatment may need to continue for months or years
- Recommended dose for giardiasis: 400 mg once daily for five days
- Recommended dose for children of 1-2 years: 200 mg as a single dose
- Not recommended for children under 1 year
Interaction
- No reported pharmacodynamic or pharmacokinetic interactions
Contraindications
- Not normally used in neonates
- Dose reduction may be indicated in children weighing less than 10 kg
- Should not be given during pregnancy or women thought to be pregnant
- No evidence to suggest dose alteration in renal, hepatic, or cardiac failure
Side Effects
- Gastrointestinal disturbances
- Headache
- Dizziness
- Changes in liver enzymes
- Reversible alopecia (rare)
- Rash
- Fever
- Blood disorders including leucopenia and pancytopenia
- Allergic shock if cyst leakage
- Convulsion and meningism in cerebral disease
Pregnancy & Lactation
- US FDA Pregnancy category: C
- Avoided in pregnancy and lactation unless potential benefits outweigh possible risks to the fetus
Precautions & Warnings
- Blood counts and liver function tests before treatment and twice during each cycle
- Breastfeeding
- Exclude pregnancy before starting treatment
- Use in the treatment of Echinococcosis only under constant medical supervision with regular monitoring
