Indications
- Adjunct to diet for reducing elevated total cholesterol levels
- Adjunct to diet for reducing LDL cholesterol levels
- Adjunct to diet for reducing apolipoprotein B levels
- Adjunct to diet for reducing triglycerides levels
Pharmacology
- Selective inhibitor of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels
- Inhibits cholesterol synthesis in the liver
- Increases hepatic LDL receptors for enhanced uptake of LDL
Dosage & Administration
- Recommended starting dose for adults: 10 or 20 mg once daily
- Maximum dose for adults: 80 mg once daily
- Pediatric dose: 10-20 mg once daily for 10-18 years old
- Dosage adjustment based on lipid levels within 2-4 weeks of initiation
Interaction
- Increased risk of myopathy with concurrent administration of cyclosporine
- Altered plasma concentrations with coadministration of antacid or colestipol
- Increased plasma concentrations with coadministration of erythromycin or oral contraceptives
- No clinically significant effect on prothrombin time with warfarin
Contraindications
- Hypersensitivity to any component of the medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

Pregnancy & Lactation
- Contraindicated during pregnancy

Precautions & Warnings
- Liver function tests before initiation of treatment and periodically thereafter
- Caution in patients with substantial alcohol consumption or history of liver disease
- Discontinue therapy if markedly elevated CPK levels occur or if myopathy is diagnosed or suspected
Use in Special Populations
- Higher plasma concentrations in healthy elderly subjects
- No clinically significant difference in LDL-C reduction between men and women
- No dose adjustment in renal dysfunction patients
- Markedly increased plasma concentrations in chronic alcoholic liver disease patients

- Symptomatic treatment and supportive measures
- Liver function tests and serum CK levels monitoring
- Hemodialysis not expected to significantly enhance drug clearance
