Pharmacology
- Potent topical corticosteroid
- Anti-inflammatory
- Antipruritic
- Vasoconstrictive properties
- Inhibition of late phase allergic reactions
- Decreased mast cell density
- Inhibition of cytokine production
- Metabolism inhibition of arachidonic acid
Dosage
- Application frequency: twice daily
- Super-high potency topical corticosteroid
- Limit treatment to 2 consecutive weeks
- Maximum weekly dose limit: 50 gm/week
- Limit courses for children if possible to five days
- Total scalp application dosage not to exceed 50 ml weekly
- Limit shampoo treatment to 4 consecutive weeks

- Creams suitable for moist or weeping surfaces
- Ointments suitable for dry, lichenified or scaly lesions

Interaction
- CYP3A4 inhibition by co-administered drugs (e.g. ritonavir, itraconazole)
Contraindications
- Rosacea, acne vulgaris, perioral dermatitis
- Hypersensitivity to the preparation
- Not indicated for infected skin lesions caused by fungi or bacteria
- Dermatoses in children under one year of age including dermatitis and napkin eruptions
Side Effects
- Prolonged use may lead to systemic absorption
- Local atrophic changes in the skin (e.g. thinning, striae)
- Possible provocation of pustular form of psoriasis
- Reports of pigmentation changes and hypertrichosis
- Signs of hypersensitivity may occur
Pregnancy & Lactation
- Limited data in pregnant women
- Topical corticosteroids in animals can cause foetal abnormalities
- Caution advised during lactation
Precautions & Warnings
- Avoid long-term continuous therapy, especially in infants and children
- Atrophic changes more likely on the face
- Avoid application to eyelids to prevent eye entry
- Hazardous in psoriasis due to several reasons
- Appropriate anti-microbial therapy for inflammatory lesions
Use in Special Populations
- Avoid long-term therapy in infants and children under 12 years
- Children more susceptible to atrophic changes
Overdose Effects
- Acute overdosage unlikely
- Chronic over-dosage may lead to hypercortisolism features
