Indications
- Relieves pain and inflammation in arthritic conditions
- Relieves pain in musculoskeletal disorders
- Treats trauma-induced pain
- Relieves pain in renal colic
Pharmacology
- Potent nonsteroidal antiinflammatory drug (NSAID)
- Has analgesic and antipyretic properties
- Inhibits prostaglandin synthesis
- Has uricosuric effects
- Lidocaine acts as a local anaesthetic
Dosage & Administration
- Adult dosage: one ampoule once daily by intramuscular injection
- Renal colic dosage: one ampoule once daily intramuscularly, may repeat after 30 minutes if necessary
- Recommended dosage for children and elderly patients
Interaction
- May increase plasma concentrations of Lithium and Digoxin
- Reports of increased risk of haemorrhage with anticoagulant therapy
- Can be given together with oral antidiabetic agents without influencing their clinical effect
- Cases of nephrotoxicity with concomitant use of Cyclosporin
- Potential of serious toxicity when used with Methotrexate
- Caution needed when considering concomitant therapy with quinolones
- Increased frequency of unwanted effects with other NSAIDs and steroids
- Inhibition of diuretic activity with various NSAIDs
Contraindications
- Hypersensitivity to Diclofenac or local anaesthetics
- Active or suspected peptic ulcer or gastrointestinal bleeding
- Incidence of hypersensitive reactions to specific excipients
Side Effects
- Mild and transient gastrointestinal discomfort, nausea, diarrhea, and bleeding
- Rare instances of injection site disorder, abscesses, and local necrosis
- Adverse effects due to Lidocaine mainly involve the CNS and are dose related
Pregnancy & Lactation
- Not recommended unless compelling reasons exist
- Lowest effective dosage should be used
- Not recommended during the last trimester of pregnancy
- Small quantities may be detected in breast milk with no expected undesirable effects on the infant
Precautions & Warnings
- Close observation for patients with severe hepatic, cardiac, or renal insufficiency or the elderly
- Renal function monitoring for patients using NSAIDs
- Discontinuation if abnormal liver function tests persist or worsen
- Monitoring of all patients receiving long term treatment with NSAIDs
