Indications
- Treatment of hypertension as substitution therapy in patients controlled with individual products
- Substitution therapy for adequately controlled patients
- Treatment of high blood pressure
Pharmacology
- Contains Amlodipine and Bisoprolol Fumarate
- Amlodipine inhibits calcium ion influx into cells
- Amlodipine reduces peripheral vascular resistance
- Bisoprolol is a synthetic, beta1-selective adrenoceptor blocking agent
Dosage & Administration
- One tablet once daily
- Recommended fixed-dose combination
Interactions
- Combinations not recommended with verapamil, diltiazem, and centrally-acting antihypertensive drugs
- Caution with strong or moderate CYP3A4 inhibitors, CYP3A4 inducers, simvastatin, Tacrolimus, Cyclosporine, etc.
- Combinations to be considered with Mefloquine, Rifampicin, Ergotamine derivatives, etc.
Contraindications
- Acute heart failure or during episodes of heart failure decompensation
- Obstruction of the outflow tract of the left ventricle
- Cardiogenic shock
- Sick sinus syndrome
- Severe bronchial asthma
- Untreated phaeochromocytoma
- Hypersensitivity to bisoprolol, amlodipine, etc.
Side Effects
- Common: Dizziness, headache, somnolence, palpitations, flushing, gastrointestinal complaints, edema, fatigue
- Uncommon: Insomnia, mood changes, visual disturbances, bronchospasm, skin discoloration, myalgia, potency disorders
- Rare: Nightmares, hallucinations, allergic reactions, hepatitis
Pregnancy & Lactation
- Not recommended during pregnancy and lactation
Precautions & Warnings
- Caution in patients with heart failure, impaired hepatic function, obstructive airways diseases
- Avoid in patients with certain conditions like bronchospasm, diabetes mellitus, AV block, thyrotoxicosis
- Careful use in patients undergoing general anesthesia
Use in Special Populations
- Caution in elderly, lack of data for pediatric use, special caution in hepatic impairment
- No dosage adjustment required for mild to moderate renal impairment
Overdose Effects
- Signs of overdose with beta-blockers
- Possible consequences of amlodipine overdose include excessive vasodilation and hypotension
- Discontinuation of treatment and supportive/symptomatic treatment recommended in case of overdose
