Indications
- Treatment of dyspeptic symptom complex

Pharmacology
- Dopamine antagonist
- Blocks dopamine receptors in Chemoreceptor Trigger Zone (CTZ) and stomach
- Restores normal motility and tone of upper gastrointestinal tract
- Facilitates gastric emptying
- Enhances antral and duodenal peristalsis
- Regulates contraction of the pylorus
- Increases esophageal peristalsis
- Enhances lower esophageal sphincter pressure
Dosage & Administration
- To be taken 15-30 minutes before meals
- Usual recommended oral dose for adults: 10-20 mg every 6-8 hours daily
- Maximum daily dose for adults: 80 mg
- Different dosage for children based on body weight
- Rectal administration in suppositories for adults and children
- Max treatment period for acute nausea and vomiting: 12 weeks
Interaction
- Antagonism with anticholinergic drugs
- Lowered oral bioavailability with antacids and antisecretory drugs
- Metabolism through CYP3A4 pathway
- Possible inhibition of CYP3A4 due to other drugs
- No potentiation of action with neuroleptics
- Suppression of unwanted peripheral effects of dopaminergic agonists
Contraindications
- Hypersensitivity to the drug
- Neonates
- Dangerous gastrointestinal stimulation conditions
- Prolactin releasing pituitary tumor (prolactinoma)
Side Effects
- Rare occurrence of transient intestinal cramps
- Rare extrapyramidal phenomena especially in children
- Potential increase in plasma prolactin level
- Possible allergic reactions such as rash and urticaria
Pregnancy & Lactation
- Limited usage during first trimester if justified by therapeutic benefit
- No evidence of increased risk of malformations in humans
- Excretion in breast milk with lower concentrations than plasma
- Nursing not recommended unless benefits outweigh risks

- Caution in children due to risk of extra-pyramidal reactions
- Caution in patients with hepatic impairment

- Caution in infants due to immature blood-brain barrier
- Caution in patients with liver and kidney disorders

Overdose Effects
- Symptoms may include drowsiness, disorientation
- Use of activated charcoal and close patient observation recommended
- Anticholinergic or antihistamines may help control extrapyramidal reactions
