Indications
- Treatment of major depressive episodes
- Treatment of panic disorder with or without agoraphobia
- Treatment of social anxiety disorder (social phobia)
- Treatment of generalised anxiety disorder
- Treatment of obsessive-compulsive disorder
Pharmacology
Escitalopram is an orally administered selective serotonin reuptake inhibitor (SSRI).
Dosage
- Safety of daily doses above 20 mg has not been demonstrated
- Administered as a single daily dose
- Usual dosage for major depressive episodes is 10 mg once daily
- Initial dosage for panic disorder is 5 mg, increasing to 10 mg daily
- Initial dosage for social anxiety disorder is 10 mg once daily
- Initial dosage for generalised anxiety disorder is 10 mg once daily
- Initial dosage for obsessive-compulsive disorder is 10 mg once daily
Administration
Escitalopram should generally be administered once daily, morning or evening with or without food.
Interaction
SSRI or related antidepressants should not be started until 2 weeks after stopping an MAOI.
Contraindications
Escitalopram is contraindicated in patients with known hypersensitivity to Escitalopram or Citalopram.
Side Effects
- Less sedating than tricyclic antidepressants
- Gastrointestinal effects like nausea, vomiting, and diarrhea
- Hypersensitivity reactions including rash, urticaria, and angioedema
- Other side effects include dry mouth, nervousness, anxiety, and headache
Pregnancy & Lactation
Physicians should carefully consider the potential risks and benefits of treatment when administering Escitalopram to pregnant or lactating women.
Precautions & Warnings
- SSRIs should be used with caution in patients with epilepsy
- Caution is advised in patients with hepatic and renal impairment
- Caution in patients with history of mania, cardiac disease, and diabetes mellitus
Use in Special Populations
- Elderly patients (>65 years of age) should start with 5 mg once daily
- Children and adolescents (<18 years) should not use Escilex
- No dosage adjustment necessary in patients with mild or moderate renal impairment
- Caution is advised in patients with severely reduced hepatic function
Overdose Effects
- Symptoms include dizziness, tremor, and agitation
- Management includes establishing and maintaining an airway, gastric lavage, and ECG monitoring
