Indications
- Treatment of bronchial asthma
- Treatment of bronchospasm
- Treatment of chronic obstructive pulmonary disease (COPD)
- Treatment of pulmonary disease with spastic bronchial component
Pharmacology
- Doxophylline is a novel bronchodilator
- Selectively inhibits phosphodiesterase 4
- Relaxes bronchial smooth muscle
- Inhibits platelet activating factor (PAF)
- Decreased affinities toward adenosine A1 and A2 receptors
- Reported to inhibit leukotriene production
Dosage & Administration
- Elderly dosage: 200 mg tablet two or three times daily
- Adults dosage: 400 mg tablet two or three times daily or as prescribed by physician
- Children dosage: >12 years of age: 10 ml syrup or 200 mg tablet two or three times daily
- Children dosage: 6-12 years of age: 6-9 mg/kg body weight two times daily
- Special release tablet available for daily dose of 400 mg
Interaction
- Not to be administered with other xanthine preparations
- Limit consumption of caffeine-containing beverages and food
- Caution with ephedrine or sympathomimetic drugs
- Concurrent administration with certain drugs may reduce hepatic clearance of xanthine derivatives
- Possible dosage adjustment with concurrent use of certain drugs
Contraindications
- Known hypersensitivity to the drug or other xanthine derivatives
- Acute myocardial infarction
- Hypotension
- During lactation

- Irritability
- Insomnia
- Tachycardia
- Extrasystoles
- Tachypnea
- Hyperglycemia
- Albuminuria
- Severe cardiac arrhythmias
- Tonic-clonic seizures
Pregnancy & Lactation
- Animal tests showed no interference with pre and postnatal growth
- Use during pregnancy should be carefully evaluated on a risk-benefit basis
- Contraindicated during lactation
- No effect on ability to drive and use machines
Precautions & Warnings
- Factors may reduce hepatic clearance with increased plasma levels
- Monitoring of blood levels recommended for certain factors
- Caution in patients with cardiac disease, hypertension, hypoxemia, hyperthyroidism, liver disease, peptic ulcer, renal impairment
- Caution in congestive heart failure patients
- No risk of addiction or dependence
Overdose Effects
- Severe cardiac arrhythmias
- Tonic-clonic seizures
- No specific antidote
- Symptomatic treatment of cardiovascular collapse may be needed
