Indications
- Replacement therapy in hypothyroidism
- Suppression of Thyroid Stimulating Hormone (TSH) levels
- Suppression of goitrogenic effects of other drugs
- Diagnostic aid in suppression tests
Pharmacology
- Contains synthetic Levothyroxine (T4)
- About 30% of T4 is converted to Triiodothyronine (T3)
- TBG is the major carrier of T4
- Long half-life in circulation
- Enterohepatic circulation of Thyroid hormones
Dosage & Administration
- Adult dose adjustments based on clinical euthyroid state and TSH levels
- Pediatric dosage based on age and weight
- Dose adjustments for cardiac disease and underlying conditions
Interaction
- Concurrent use with tri/tetracyclic antidepressants
- Interaction with antidiabetic or insulin therapy
- Reduced serum digitalis glycoside levels
Contraindications
- Untreated subclinical or overt Thyrotoxicosis
- Acute Myocardial Infarction
- Uncorrected Adrenal failure
Side Effects
- Hyperthyroidism symptoms due to therapeutic overdose
- General, CNS, musculoskeletal, cardiovascular, respiratory, gastrointestinal, and dermatologic reactions
Pregnancy & Lactation
- Pregnancy Category A
- Increase in Levothyroxine requirements during pregnancy
- Caution during administration to nursing women
Precautions & Warnings
- Adrenocortical insufficiency in pituitary gland function
- Careful dosing for patients with cardiovascular disease
- Adjustment of dosage with marked change in body weight
- Monitoring serum levels of T3, T4, and TSH
