Indications

• Treatment of Parkinson's disease
• Relief of parkinsonism symptoms
• Management of tremor
• Treatment of dysphagia
• Treatment of sialorrhea
• Management of postural instability

Description

• Levodopa as metabolic precursor of dopamine
• Carbidopa as a dopa decarboxylase inhibitor

Dosage & Administration

• Initiation dosage for 100/10 mg tablet
• Initiation dosage for 250/25 mg tablet
• Titration upward for optimum dosage
• Maintenance dose individualization

Interaction

• Symptomatic postural hypotension with antihypertensive medicines
• Adverse reactions with tricyclic antidepressants
• Decrease in bioavailability with ferrous sulphate
• Reduced therapeutic effects with dopamine-2 receptor antagonists
• Reversed effects with phenytoin and papaverine
• Severe orthostatic hypotension with selegiline

Contraindications

• Hypersensitivity to Carbidopa and Levodopa
• Narrow-angle glaucoma
• Risk of activating malignant melanoma

Side Effects

• Central neuropharmacologic activity of dopamine
• Dyskinesias, including choreiform and dystonic movements
• Nausea, syncope, chest pain, anorexia
• Palpitation, orthostatic effects, hypertension
• Gastrointestinal effects, haemotologic effects
• Hypersensitivity reactions, nervous system effects
• Respiratory effects, skin effects, urogenital effects

Pregnancy & Lactation

• Unknown effects on human pregnancy
• Potential hazards in women of childbearing potential
• Unknown excretion in human milk
• Potential for serious adverse reactions in infants

Precautions & Warnings

• Not recommended for medicine-induced extrapyramidal reactions
• Discontinuation of Levodopa before starting Carbidopa-Levodopa
• Observation for development of depression with suicidal tendencies
• Caution in patients with cardiovascular, pulmonary, renal, hepatic or endocrine disease
• Special care for patients with a history of myocardial infarction

Use in Special Populations

• Not recommended for patients below the age of 18 years
