Indications
- Acute bacterial exacerbation of chronic bronchitis in adults
- Acute maxillary sinusitis
- Community-acquired pneumonia
- Pharyngitis or tonsillitis
- Uncomplicated skin and skin structure infections
- Acute otitis media in pediatric patients
- Treatment and prophylaxis of disseminated mycobacterial infections
- Helicobacter pylori infection and duodenal ulcer disease in adults
Pharmacology
- Inhibits microsomal protein synthesis
- Binds to the donor site on the 50S subunit of the bacterial ribosome
- Active against most Gram-positive bacteria and Chlamydia
- More acid-stable than Erythromycin
- Twice the activity of Erythromycin against H. influenzae
Dosage & Administration
- Adult dosages for different indications
- Combination dosing regimens for H. pylori infection
- Recommended daily dosage for children
- Administration in renal and hepatic impairment
Interaction
- Contraindicated with specific medications such as HMG-CoA reductase inhibitors and antipsychotics
Contraindications
- Known hypersensitivity to clarithromycin or macrolide antibacterial drugs
- History of cholestatic jaundice or hepatic dysfunction associated with prior use of clarithromycin

Pregnancy & Lactation

- Avoid in patients with known QT prolongation or receiving specific drugs
- Discontinue if signs and symptoms of hepatitis occur
- Increased risk of all-cause mortality in patients with coronary artery disease
Use in Special Populations
- Safety and effectiveness in pediatric patients 6 months and older
- Safety in pediatric patients 20 months and older with advanced HIV infection
- Safety not established in MAC patients under the age of 20 months
Overdose Effects
- Gastro-intestinal symptoms in large amounts
- Altered mental status and paranoid behavior in extreme cases
- Prompt elimination of unabsorbed drug and supportive measures for treatment
