Indications
- Treatment of major depressive disorder (MDD)
Pharmacology
- Enhances central noradrenergic and serotonergic activity
- Acts as an antagonist at central presynaptic α2-adrenergic inhibitory autoreceptors
- Potent antagonist of 5- HT2 and 5-HT3 receptors
- Has no significant affinity for the 5-HT1A and 5-HT1B receptors
- Moderate peripheral α1-adrenergic antagonist
- Moderate antagonist at muscarinic receptors
Pharmacokinetics
- Rapid and well-absorbed after oral administration
- Mean half-life of elimination is 20-40 hours
- Displays linear pharmacokinetics within the recommended dose range
- Extensively metabolized and eliminated via the urine and faeces
Dosage & Administration
- Recommended starting dose of 15 mg/day
- Effective dose range of 15 to 45 mg/day
- Recommended to be administered in a single dose, preferably in the evening or prior to sleep
- Dose changes should be made at intervals of less than 1 to 2 weeks
Interaction
- Clinically significant drug-drug interactions with Monoamine Oxidase Inhibitors (MAOIs) & other serotonergic drugs
- May interrupt the metabolism or activity of Carbamazepine, Phenytoin or Cimetidine
- Patient should avoid Alcohol & Diazepam
Contraindications
- Contraindicated in patients with a known hypersensitivity to Mirtazapine or to any of the excipients
- Concomitant use with a MAO inhibitor is contraindicated
- Should not be used within 14 days of initiating or discontinuing therapy with a MAOI

Pregnancy & Lactation
- Pregnancy Category-C
- Patients should be advised to notify their physician if they become pregnant or intend to become pregnant during Mirtazapine therapy

Precautions & Warnings
- Be alert to the emergence of certain behavioral changes
- Warned about the risk of developing agranulocytosis
- May impair judgment, thinking, and particularly, motor skills
