Indications
- Treatment of erectile dysfunction in men

Description
- Selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5)
- Works by increasing levels of cGMP in the corpus cavernosum, resulting in smooth muscle relaxation and increased blood flow to penile tissues
Pharmacology
- Mechanism of action involves release of nitric oxide in the corpus cavernosum during sexual stimulation
- Enhances the effect of nitric oxide by inhibiting PDE5
- Rapidly absorbed after oral administration with a mean bioavailability of 41%
Dosage & Administration
- Recommended starting dose for erectile dysfunction is 50 mg taken approximately 1 hour before sexual activity
- Maximum recommended dosing frequency is once per day
Interaction
- Metabolism primarily mediated by cytochrome P450 (CYP) isoforms 3A4 and 2C9
- Inhibitors of CYP3A4 and CYP2C9 may reduce drug clearance
- Inducers of CYP3A4 and CYP2C9 may increase drug clearance
Contraindications
- Contraindicated in patients with hypersensitivity to any component of the medication
- Contraindicated in patients using organic nitrates
Side Effects
- May include face edema, chest pain, vomiting, anemia, myalgia, insomnia, dyspnea, urticaria, and prolonged erection
- Serious cardiovascular, cerebrovascular, and vascular events reported post-marketing
Pregnancy & Lactation
- Pregnancy category B
- Not indicated for use by women
Precautions & Warnings
- Caution advised when co-administered with alpha-blockers
- Consideration advised for patients with bleeding disorders, active peptic ulceration, and anatomical deformation of the penis
- Not generally recommended for men for whom sexual activity is inadvisable due to underlying cardiovascular status
Overdose Effects
- In cases of overdose, standard supportive measures should be adopted as required
- Renal dialysis not expected to accelerate drug clearance
